Total Knee Replacement and the Role of Physical Therapy

Total knee replacement (TKR) surgery is one of the most effective treatments for advanced osteoarthritis and other age-related changes in the knee joint. Because the knee is a major weight-bearing joint, it’s especially vulnerable to years of wear and tear. Over time, this can lead to pain, stiffness, swelling, and difficulty with activities such as climbing stairs, squatting, or walking on uneven ground.

Preparing for Surgery: “Prehab”

In many cases, your surgeon may recommend physical therapy before a total knee replacement. This “prehab” phase helps strengthen your knee, improve flexibility, and optimize function leading into surgery. The stronger and more mobile your knee is beforehand, the smoother your recovery is likely to be. In fact, some people improve so much during prehab that surgery may be delayed—or even avoided altogether.

Recovering After Surgery

Once you’ve had your knee replacement, physical therapy becomes essential to regaining your independence. Early rehab focuses on restoring your range of motion. Your therapist will work with you to straighten your knee and help you achieve a bend of at least 120 degrees. Hitting these milestones allows you to walk normally, climb stairs, and sit or stand comfortably.

At the same time, therapy emphasizes activating your quadriceps, the muscle group in the front of your thigh. Strong quads are crucial for supporting your surgical leg, improving balance, and reducing reliance on assistive devices.

As you progress, your therapist will guide you through strengthening and stability exercises for your entire surgical leg. These steps help restore confidence and function for everyday activities—like getting up from a chair, climbing stairs with ease, or keeping up with your kids or grandkids.
